Cerebrolysin: What the Evidence Actually Shows

What it actually is

This is worth stating plainly because the marketing tends not to. Cerebrolysin is not a defined molecule. It is a preparation of low molecular weight peptides and amino acids obtained by enzymatic breakdown of purified pig brain proteins. Batch-to-batch consistency is inherently harder for a preparation of that kind than for a synthetic peptide.

What the evidence shows

It has been used clinically for decades in several countries, and there is a substantial body of published work. There are also systematic reviews, including Cochrane work on acute ischaemic stroke, that have not found convincing evidence of benefit on important outcomes, alongside concerns about the quality and independence of some supporting studies.

The fair summary is that this is contested rather than settled, in either direction.

The origin question

A product derived from animal brain tissue carries considerations that synthetic peptides do not, including theoretical transmissible agent concerns and immunogenicity. Approved use elsewhere comes with manufacturing controls. Gray market supply does not.
