When muscle is damaged by mechanical loading, the IGF-1 gene is spliced differently, producing a variant with a distinct C-terminal peptide. That variant appears to activate satellite cells, which are the muscle stem cells responsible for repair. It is a locally produced, locally acting signal.
The key word is local. MGF is expressed in the damaged tissue, acts there, and is short lived. Injecting a synthetic version into the bloodstream, or into a muscle, is a fundamentally different proposition from the physiology being cited, and it has not been shown to reproduce it.
PEG-MGF exists to extend circulation time, which arguably moves further from the natural pattern rather than closer to it.
